EEA (EØS) 4.1 - application for document PD U1

Form EEA 4.1 is used to apply for document PD U1. PD U1 can be used for transferring Danish periods of employment and unemployment insurance, if you are to apply for unemployment benefits in another EEA country.

If you were a member of a Danish unemployment insurance fund while you worked in Denmark, please forward the application form to your unemployment insurance fund. If you were not a member of a Danish unemployment insurance fund, please send your application form to the Danish Agency for Labour Market and Recruitment.

You can ask your previous employer to submit the required information on virk.dk or to complete application form EØS 7. Your previous employer must provide information to your unemployment insurance fund or the Danish Agency for Labour Market and Recruitment. If you have had several employers, each of them will have to give information about your previous employment.

If your previous employer does not provide this information, you can complete form EEA 4.1 and send it to your unemployment insurance fund or the Danish Agency for Labour Market and Recruitment. The unemployment insurance fund or the Danish Agency for Labour Market and Recruitment must subsequently collect information from your previous employer.

It is important that you state the address where you want to receive the PD U1 document.
